I, the parent or legal guardian of the above-named participant, acknowledge that participation in a touch football tournament involves physical activity and the risk of injury. I knowingly and voluntarily assume all risks associated with participation. I hereby release, waive, and hold harmless the tournament organizers, staff, volunteers, referees, sponsors, and facility from any and all claims, demands, or causes of action arising out of participation in this event.
